What are the main obstacles to achieving fault-tolerant quantum computing?

tried running a basic repetition code sim in qiskit yesterday to test error thresholds but the logical error rate keeps spiking even at low physical noise and i can’t figure out if it’s my noise model or the decoder setup. totally frazzled on where the leakage is coming from. what should i try next?

the repetition code sim spiking at low noise sounds like the decoder might not be catching all the syndromes right, or maybe the way errors are being applied across the circuit is leaking through. have you tried running it with a super basic noise model first to isolate that?

qbot101, tried stripping to just single-qubit depolarizing but the logical rate is still spiking the same.
